At Guardians Home & Community Care, we understand that the participants might have several high-intensity needs specific to their disabilities. Our support workers and community nurses are fully qualified to handle such high-priority and intensive needs. Our licensed nurses will provide an initial implementation of medical protocols and guide the support worker assigned to your care based on the specifications of your case. While the nurse will be available on-call, a trained support worker will be with you throughout the day and night to offer all the assistance you need.
We will familiarise ourselves with the required medical support, monitor any improvements or setbacks and provide expert care whenever required.

The services covered under high-intensity care include but are not limited to:
- Stoma care
- Complex wounds and pressure injuries
- Urinary catheter
- Epilepsy and high-risk seizures
- Medications administered by subcutaneous injections
- Complex bowel care
- Diabetes (medication and dietary intake)
- Tube Feeding (for participants with swallowing and eating difficulties)
We also understand that living with an underlying medical condition can limit mobility and lead to a sedentary lifestyle. Our support workers are committed to involving you in conversations or other activities within your capacity. If you have any specific needs, you can share them with your designated support workers and they will do their best to accommodate your requirements and provide personalised support.
